Deleted phone numbers reappear after page refresh

Example is user 100519975. I deleted the phone number multiple times in both the https://pcmedlink.org/country/roster and the https://pcmedlink.org/admin/users/232/edit but the phone number still remains for the user.

I haven't had this issue moving forward, so it's possible that it may have been an issue with multiple browsers being open at one time.
