Rolls up #135, #136, #137, #138, and #139:

• In-process enforcement of enum, text(max:), integer(min:/max:) (Server: enum/length/min-max constraints are unenforced in-process; violations return 502 instead of 422 #133) and table-wide unique on @tenant(root) schemas (Migrations: unique on a @tenant(root) schema is scoped to _tenant and never enforced #134); acton-service 0.39.0
• File routes enforce record policy and field permissions before storage access and hooks
• apply / migrate --execute persist metadata-only schema changes
• Conditional entity mutations with atomic record revisions (PostgreSQL, opt-in via apply --prepare-record-revisions)
• Atomic create-intent reconciliation for lost create responses (PostgreSQL)

Gate on the merged tree: workspace clippy 0 warnings (incl. surrealdb feature), 2424 nextest tests passed.
