I have a modified version of your project.
I can only thank you for this project which was very instructive for me.
So after Update 1 or some earlier update of SSDT they changed project type string from SQLDB to SSDT and something else happened and now my project template does not appear in project creation dialog.
I think you might have the same issue.
Do you know how to fix this?
